A retrospective review was conducted of the Wisconsin Disease Reidentification Program (DReIP), a voluntary state-wide disease registry using an EHR. Descriptive statistics were used to report data on disease reidentification among community members in the DReIP. A total of 19,636 patients had at least one outpatient visit in Wisconsin during 2005-2010. From 2005-2010, there were 1,109,493 inpatient stays, 12,127,926 outpatient visits and 228,819 emergency department (ED) visits in Wisconsin. The frequency of disease reidentification (of record in the EHR) among the 925 patients in the DReIP who had at least one hospitalization in Wisconsin was 15.8%. The frequency of reidentification among the 1,109,493 patients who had at least one inpatient visit in Wisconsin was 3.4%.
